    information.

    party) when you submit patient forms.

     We use this information to ensure patient
    safety, provide dental services, process insurance claims, provide customer
    service, and detect and investigate insurance fraud or other illegal activity.

  8. Professional
    and Employment Information

    What we
    collect:
     If
    you apply for a job with us, we will collect any information you submit about
    previous employers, work experience, your resume, or other information you
    provide with your job application.

delete cookies by going to the preferences, tools or options menus of your
